Chronic Disease Self-Management Program
The “Take Action for a Healthier Life” Chronic Disease Self-Management Program is an evidence-based program that was developed at Stanford University. This program is 2.5 hours, once a week, for six weeks. Take Action for a Healthier Life is open to anyone in the community who is dealing with a chronic condition and wishes to enhance their ability to manage their own health care. The program is also open to family members, friends, and caregivers of those with chronic conditions. Participants will learn how to:
- Work with their Doctor
- Goal Setting
- Problem-solving
- Brainstorming
- Communication Skills
- Manage Fatigue and Frustration
- Manage Symptoms
- Relaxation Techniques
- Eat well/Develop Exercise Program

*If you would like to keep the book and supplies the cost of the program will be $40.
The program will be facilitated by Charlotte Crawford and Christi Hayden who have both been trained as Master Trainers for the Chronic Disease Self-Management Program. Once they complete two six week programs for the community they will be able to hold leader trainings and train others to facilitate the program.
